Prior to your service appointment, you will be required to fill-out an intake form(s) for each of the services you booked with us along with a Covid-19 questionnaire, this can take up to 15 minutes.
Anything past 10 minutes is considered late. In the event you do arrive late for your appointment, we will try our best to accommodate you.
